1. Interest Form
   • Please fill out this brief form to get started today to become a Special Olympics Indiana athlete! We are so excited to have you join us!
2. Athlete Registration
   • Once you fill out the athlete interest form, your county coordinator will be in touch with you and send you an online link to the athlete registration. They will send you a page for general demographic and contact information!
3. Medical Form
   • Upon completion of your athlete registration, you will receive a blank medical exam form. Please take this to a licensed practitioner, and return to your county coordinator.

*Special Olympics is open to individuals with intellectual disabilities who are 8 years of age or older. There is no maximum age limit for participation. All sports are free. Special Olympics never charges for an individual to participate in our sports programs.
